Scheduling and Preparation

Booking a housecall visit is simple and tailored to your needs. Our team gathers important health information during scheduling, including current symptoms, medications, allergies, and any equipment needs. We also take note of accessibility considerations to ensure your clinician arrives prepared. The day before your visit, you’ll receive a confirmation with your provider’s name, estimated arrival window, and tips on what to have ready—like your medication list or any recent test results.

Arrival and Safety Review

When your clinician arrives, they will introduce themselves, sanitize their hands, and do a quick environmental scan for safety risks such as tripping hazards or improperly stored medications. This helps us identify ways to support aging in place more safely. You’ll have ample time to discuss health concerns without the pressure of a rushed visit. Initial housecalls typically last 45 to 60 minutes, allowing for a thorough review of your health and care goals.

Medical Assessment and On-the-Spot Testing

Housecalls provide the same level of medical evaluation you’d receive in a clinic. Your provider may check vitals, perform a focused physical exam, and carry out bedside testing such as blood glucose, INR, urinalysis, or strep testing. We can also administer injections, IV hydration, wound care, or prescribe and deliver equipment such as walkers, nebulizers, or blood pressure monitors—all without leaving your home.

Creating a Personalized Care Plan

After the exam, your provider will create a personalized treatment plan. This might include changes to medication, dietary recommendations, referrals for physical therapy, or the setup of remote monitoring devices. We take time to explain every step and ensure you understand when and how to follow up. If a caregiver is present, we welcome them into the conversation for support and clarity.

Before we leave, you’ll receive both a printed and digital summary of your visit, including what to watch for and how to reach us if anything changes.

Follow-Up and Ongoing Support

Within a day, your visit notes will be uploaded to our secure patient portal, and prescriptions sent to your pharmacy. If labs were drawn, a nurse will follow up to discuss results and help coordinate next steps. For patients with ongoing needs, we offer regular housecalls to help manage chronic conditions and reduce the risk of hospitalization.
